REGISTRATION CHECKLIST

• 2025 FLYFCL Registration Form
• Copy of NYS School Health Examination Form (Exam dated 8/2/24 or after, OK for full activity, signed and dated by a Dr.)
• Copy of Player’s Birth Certificate
• Copy of Player's Last Report Card (or, for those entering Kindergarten, a copy of the letter school sends welcoming them to school/their class)
• Media Release 
• Volunteer Form
• Payment

Teams Are Determined by Grade

Flag Football: K - 2nd Grade

C Team Football: 3rd & 4th Grade

B Team Football: 5th & 6th Grade

Flag Cheer: K - 2nd Grade

C Team Cheer: 3rd - 5th Grade

B Team Cheer: 6th - 8th Grade

Cost

 Flag Football - $100/player

All Cheer Teams - $125/cheerleader

B and C Team Football - $150/player

$200 max/family

Outside of the Caz/DeRuyter School District: $200/sport; $250 Max/Family
